What Issue & Remediation does

Registers

  • Anyone can open a register and run it: its own severities with target days, categories, custom attributes and visibility
  • Everyone in the organization takes part — an issue or an action can be assigned to any member, no grant needed
  • Defaults for new registers set once in Issue Settings

Issues

  • A due date from the severity, moved only with a reason that is logged
  • An action plan whose actions have their own assignees and due dates, each on that person’s My Tasks
  • Resolving needs every action done and evidence attached; closing as not an issue or accepted takes a note

Escalation and closure

  • A ladder per register: overdue by so many days, escalate to the administrator, the Module Admins or a named person — worked out live, never stale
  • A validator who is never the person who resolved it, who validates or returns it with a note
  • A dashboard of overdue, escalated, ageing and throughput, and six printable reports with CSV

Issue & Remediation: questions and answers

Who can be assigned an issue?
Anyone in the organization. Issue & Remediation is open to every member, so an issue or an action can go to whoever runs the affected process without anyone granting them access first.
How are overdue issues escalated?
Each register has an escalation ladder: when an issue is overdue by a set number of days it escalates to the register's administrator, the Module Admins or a named person. The level is worked out live from the due date, so it is never stale.
Who closes an issue?
Resolving an issue needs every action done and evidence attached. A validator — never the person who resolved it — then validates the fix or returns it with a note.
